When a natural death occurs at home, it is proper to contact the funeral home after a healthcare professional has pronounced the deceased. After you call us, we will transfer the deceased to the funeral chapel.
Things to Know:
- Social Security Number
- Date of Birth
- Father’s Name
- Mother’s Maiden Name
- Birthplace
- Education
- Previous Employer Information
Things to Bring with You:
- Recent photograph
- Military Documents/DD214
- Cemetery Deed
- Clothing for the Deceased if not using a shroud
- Anything You Would Like Placed Inside the Casket
If Death occurs at a Hospital
If death occurs at the Hospital, the Hospital will contact the family and the family will contact the Louis Suburban Chapel Jewish funeral home.
If Death occurs at a Nursing Home
If death occurs at a nursing home, they will generally have a funeral home on file and they will contact the funeral home.
If Death occurs in Hospice
If death occurs during hospice, the family calls hospice nurse and the nurse contacts the funeral services company.
If Death occurs in Another State or Country
If death occurs in another state or country, call Louis Suburban Chapel and we will make all the necessary arrangements to have the deceased transferred to our funeral home, in conjunction with a local funeral home.
